Pre-registration & pre-payment are required – please be sure to upload any previous vaccine records when you register – forms will be pre-printed for your convenience so we are unable to accept rabies certificates or distemper history during the clinic.

We encourage everyone to use our convenient online form to pre-register for vaccine clinics. This helps keep our phone lines open for emergencies. *Registration is available by phone during hospital hours for those without access to the internet by calling 508-637-1333. *A $5 manual registration fee applies.

IMPORTANT – If this is your puppy or kitten’s first vaccines, please check the minimum age requirements below.  In order to keep our vaccine clinic prices low, if a pet’s age falls below the minimum age for the clinic day, registration fees may only be applied to a future clinic date or vet appointment.

Minimum Age Requirements for Vaccine Clinic:
Rabies: 12 weeks
Distemper: 6 weeks
Microchip: 8 weeks

    I hereby authorize the veterinarian to examine, prescribe for, or treat the above pet. I assume responsibility for all charges incurred in the care of this animal.

    I understand that this is not a full and complete exam and for the overall health of my animal(s). Your animal(s) have been examined to determine the appropriateness of vaccinations selected. A comprehensive exam should be performed yearly by a Veterinarian.

    I have read and understand the following: Vaccine Reactions. Vaccine reactions can happen. If you think your pet is having a reaction to any vaccine given, please seek immediate medical assistance from your regular veterinarian. Please watch for symptoms of vaccine reaction such as Lethargy, vomiting, fever, swelling, and difficulty breathing.

    **Please double check to be sure all information is correct. **Please be sure you are available for the entire timeframe of the clinic, you will be assigned an arrival time slot. Vaccine clinic fees are non-refundable.
